What is the Cancer and Palliative Care Team?

The Cancer and Palliative Support Team, in partnership with Macmillan Cancer Support, provide a holistic and flexible home based service of cancer and palliative care and bereavement support.

Our Cancer and Palliative Care Support Workers provide emotional support, practical support and bereavement support working in coalition with health workers, and carers, to provide a package in which each member is regarded as an integral part of the team.

Service Objectives of the Cancer and Palliative Care Team

What types of care do we provide?

HEALTH TASKS;

Our Carer Support Workers provide all aspects of personal care to clients and assist the District Nursing Service in tasks such as the changing of dressings. The staff work to the District Nurse's Care Plan and make notes in this. This ensures effective liaison between the whole care team involved with a client.

SOCIAL CARE TASKS;

Our Carer Support Workers provide practical support to the carer(s) or family of the person living with cancer (for example shopping, cleaning and the preparation of meals.) This can relieve some of the pressure on carers at a very difficult time.

EMOTIONAL SUPPORT;

This is envisaged as being a vitally important aspect of the work of the project. Staff are able to offer a listening ear to the patient and their family. We offer a 'signposting' service to local resources and support organisations to carers aiding the re-organisation of their priorities as they face often bewildering and stressful situation

CONTINUITY OF CARE;

Information is the common thread linking care from one provider to another and from one 'event' to another. Information can be illness or person focused. Trent Crossroads not only focuses information on the medical condition, but also knowledge about the patient's preferences and values.

Management is especially important with the complexity of Cancer and Palliative cases. East Midlands Crossroads services are delivered in a professional and carer focussed manner. Shared or mutual learning between providers, that East Midlands Crossroads actively promote facilitate management continuity. Our services are centred around quality, predictability, consistency and flexibility for both both patients and providers.

BEREAVEMENT SUPPORT GROUP;

The team hold an informal 'Carer Led' group support meeting for bereaved carers once a month at the East Midlands Crossroads office. (follow the "Bereavement Group" link for further details on this group)
